What is Palliative Home Care?
Palliative care is a medical approach dedicated to patients with incurable diseases, aimed at improving quality of life and alleviating suffering.
This care focuses on controlling pain and physical symptoms, but also offers emotional, social and spiritual support. The approach is humane and interdisciplinary, involving physicians, nurses, psychologists and social workers to ensure that patients and their families receive maximum comfort and respect during a difficult phase of life.

Who it is addressed to
Palliative Home Care is for people with incurable diseases, including cancer cases, heart disease, cerebrovascular disorders, liver disease, and neurodegenerative diseases such as ALS. Suitable for patients of all ages, this care can be adjunct to active therapies in both the early and advanced stages of the disease.
How to activate
To activate Palliative Home Care in Sicily, the General Practitioner or the doctor of the Hospitalization Operative Unit must send a request to the patient's District PUA, also via email. The PUA performs a multidimensional assessment of the district UVM/UVP. The necessary forms include the request for the multidimensional evaluation, the "SVAMA" health form filled out by the General Practitioner, the request for the Regional Prescription Book, the anamnestic clinical form, the self-certification declaration and copies of identification documents. Any health documentation of the Patient is also required.
